Why this answer
The CBP Form 7501 instructions cited by CBP contain the appendix that prescribes how a manufacturer identification code is constructed: the two-letter ISO code of the manufacturer's country, the first three characters of each of the first two words of its name, up to four digits taken from the largest number on the street address line, and the first three letters of the city name. Applying that formula to the invoice's Italian manufacturer produces IT plus SIG plus COY plus 1640 plus SMY, option C. Option D fails because it swaps in 4000, a number drawn from elsewhere on the invoice rather than the street address, and garbles the name segment. Watch for: Building the numeric segment from a postal code or invoice number instead of the largest street-address number.
